A Certificate of Insurance (COI) is a document that provides proof of your insurance coverage to a third party — such as a lender, landlord, general contractor, client, or government agency. It summarizes your policy information including coverage types, limits, and effective dates.

  • A general contractor requires you to show general liability coverage before working on a job site.

How to Request Your Certificate

Because certificates often need to name a specific certificate holder and may require an additional insured endorsement, they are processed through our office — not directly through the carrier portal. Fill out the form below or call us at (423) 636-3743 with the following information:

  • Your name and policy number (if known)
  • The name and address of the certificate holder
  • Whether they need to be listed as an additional insured
  • The date the certificate is needed
  • Any specific language or requirements from the requesting party

Turnaround Time

No coverage changes are effective until confirmed in writing by your carrier. A certificate of insurance is evidence of existing coverage and does not modify your policy terms.
